Lastly, I applied the evidence gathered in order to come up with the procedures. I used the case of a patient who is suffering from dyspnea. Sharma & Bakshi, 2017, define dyspnea as a condition that is characterized by shortness of breath. Thus, the first element in the evidence-based care plan is to assess the situation. This involves an assessment of the patient’s condition. Lundell et al., 2015, points out that the following conditions are likely to be observed in the assessment: abnormal sounds in the breath, restlessness, and increased heartbeat, whizzing, coughing, suffocation, and a feeling of tightness in the chest.

The secondelement in the care plan is diagnosis. The patient will be required to perform several easy physical exercises to assess the behavior of his breathing. More so, the doctor can also perform chest x-rays to find a better and specific treatment of the condition. The use of electrocardiogram may also serve an advantage in showing whether there are signs of heart-related problems like a heart attack. Also, the patients it is important to use interrogation in order to identify how the condition started, how severe it is, and how frequently it occurs (Nguyen et al., 2013). Nurses and health providers stand a good chance to share with each other the diagnosis remotely.

When issuing the treatment that is based on evidence, particular results will be expected from the patient. Given that the patient is remotely located, in order to save resources, labor, and time, only one nurse can monitor the condition of the patient at home while the others remain in the health center. He will keeping updating them on the progress of the patient  (Nguyen et al.,2013) They will all have a talk on which appropriate evidence-based decision should be done next.

The forth element in the evidence based care plan is intervention. This involves the use of evidence-based treatment, experience, and critical thinking skills to help the patient reach his/her desired goals of recovery. Also, intervention serves to eliminate or to reduce all the risk factors that are associated with dyspnea. Through the use of technology, the health practitioners get to share with other from different places on which appropriate decision to take. This is in regard with the assessed and diagnosed condition of the patient.

The sixth element in the evidence care plan is rationale. The rationale relates with assessment, diagnosis, and intervention. It entails the use of facts gathered in order to identify and discuss the nature of the dyspnea. It involves retrieving logic from the manifestations of the patient. The last element is evaluation. This involves evaluating the progress of the patient (Nguyen et al., 2013).Through the use technology, health providers get to participate jointly in the care plan.

Strategic involvement of the interdisciplinary team in the care plan can add a lot of benefits in this context of remote nursing. This because of the fact that interdisciplinary collaboration involves nurses and health care provides working in different disciplines working together under common purpose that involves setting goals, making decisions, sharing responsibilities and using the resources together. The collaboration is important because it increase engagement of the patients which in returns yields better recovery of patient. For example, older people have intertwined complex conditions that are related with physical, social and psychological disorders. Lastly, interdisciplinary collaborations should have transparent and efficient communication. Open communication will serve to encourage genuine participation among the members in the team.This will help to reduce errors which might be incurred in the process.
